We see patients of all ages, although many of our patients are elderly and/or have multiple comorbidities and complex health histories.
Role Description
This is a part-time role for a Nurse Practitioner in Calgary, AB with flexible and negotiable hours. Patients may be seen in the home, in our office, or virtually. The Nurse Practitioner will be responsible for assessing, diagnosing, and treating patients, providing ongoing care, and collaborating with other healthcare professionals. Duties also include conducting health assessments, developing and implementing care plans, and educating patients and their families on health management and disease prevention.
Qualifications
• Ability to assess, diagnose, and treat patients effectively and independently
• Strong patient care and personal skills
• Experience in developing and implementing care plans collaboratively with patients and family members
• Excellent communication and patient/family education skills
• Ability to collaborate with interdisciplinary teams
• Ability to adapt to new environments
• Comfortable working in various locations, as the majority of our clients are seen via house calls.
• Master’s degree in Nursing, valid Nurse Practitioner license
• Experience in primary care, urgent care, palliative care, or home healthcare is a plus
• Valid driver’s license and reliable transportation
