Job Summary
We are seeking a highly skilled and dedicated Senior Healthcare Professional to join our team in a part-time capacity. As a key member of our interdisciplinary team, you will play a vital role in providing exceptional care to our residents.
• *About the Role**
As a Senior Healthcare Professional, you will be responsible for delivering comprehensive primary care to residents, including performing health assessments, developing person-centered care plans, and collaborating with residents to achieve their healthcare goals. You will also participate in research, education, and leadership initiatives to improve resident care outcomes and advance the profession.
• *Responsibilities**
• Provide direct care to residents, including health assessments, medication management, and procedures within your scope of practice
• Develop and implement person-centered care plans in collaboration with residents and interdisciplinary team members
• Conduct research and participate in quality improvement initiatives to enhance resident care outcomes
• Teach and coach interdisciplinary team members on clinical care management and best practices
• Collaborate with senior leadership to develop and implement strategies to optimize illness and injury prevention, health promotion, and restorative care activities
• *Requirements**
• Master’s degree in nursing or related field
• Current registration with relevant professional college (e.g. NPA)
• Minimum 2 years of experience as a nurse practitioner in a long-term care setting
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of an interdisciplinary team
